SUBMITTING THIS FORM WILL NOT SCHEDULE A BOARDING RESERVATION
This form is only used to enter instructions on an already scheduled reservation and will not reserve a room for your pet. Please contact us 863-324-6964 or to schedule a reservation.
If you would like to help speed up the process of checking your pet into boarding you may submit the form below. Our receptionists may go over any information you have submitted to make sure everything is accurate when dropping your pet off. Please submit this form at least 24 hours before check-in so a stay member has time to enter into their reservation.

Although, OSAH does provide all needed bedding, toys, food and water dishes, we understand that sometimes you want to bring a "touch of home" with your pet as they board. Please keep in mind that while we do our best to return items left with your pet, occasionally items can be misplaced, soiled or damaged while here. If you would like to leave any personal belongings, please provide a detailed list and description below and avoid large items if possible.
If your pet is boarding in Luxury, a daily additional walk, playtime or pool time are all included. If your pet is staying in our "Standard" boarding, these items may all be added on at an additional cost.

One of the advantages of boarding your pet at Orchid Springs Animal Hospital is that veterinarian attention is readily available should the need arise. If your pet becomes ill we will call the emergency number listed above regarding your pet's symptoms, treatment options, and estimate of additional cost. Please note that if your pet presents with a life threatening or severe quality of life emergency, and we are unable to reach anyone on the listed contacts, we reserve the right to treat the patient accordingly at the doctor's discretion to save their lives and make them comfortable until your return at your expense. We will keep the cost as basic as possible, but in the best interest of your pet and in the interest of humane compassion, we will treat them if needed.
